The prize is awarded by the Sponsor and applied as twelve (12) consecutive monthly credits of up to $149 each, to a total of $1,788, toward the winner’s own CoreAge Rx plan. • Each credit covers the winner’s CoreAge Rx charges up to $149 in that month. Any amount above $149 in a given month is the winner’s responsibility. • The twelve months run consecutively from the date the first credit is applied. Unused credit in any month does not roll over. • The credits are not transferable, have no cash value, and cannot be redeemed for cash or exchanged for any other product. • If the winner’s CoreAge Rx plan ends, is paused, or is not approved by a clinician before the twelve months are complete, the remaining credits are forfeited and are not paid out in cash or substituted. • The prize covers plan charges only. Lab work, shipping surcharges, and any other cost not billed as part of the plan remain the winner’s responsibility. • The prize does not include, and cannot be exchanged for, any prescription medication. Whether the winner is prescribed anything is decided by a licensed healthcare provider, independently of this giveaway and of the Sponsor. • CoreAge Rx’s own terms of use, and any terms it applies to the winner’s account, govern the underlying service. Where those terms and these differ on anything concerning the service itself, CoreAge Rx’s terms control. • Taxes on the prize are the winner’s responsibility, as set out in the Official Rules.
